Adding a father to a birth certificate is a two-step process. First, you must establish paternity and then submit the correct form to the Texas Vital Statistics office.

STEP 1: ESTABLISH PATERNITY

In order to add a father's name to a birth certificate, he must first be established as the child’s legal father.  Read more on our Paternity page.

STEP 2: COMPLETE THE TEXAS VITAL STATISTICS FORM VS-170 - APPLICATION BASED ON PARENTAGE
