We prefer that you file your general complaint via our General Complaint online form, but if you are unable to do that, you may call our Constituent Affairs line at 1-800-621-0508, and we will mail you a complaint form that you may return by mail.

Only General Complaints may be submitted by mail. Privacy and Charitable complaints must be filed online.

DO NOT INCLUDE any sensitive or confidential information in your complaint such as your date of birth, Social Security Number, government-issued ID, or financial account numbers. 

Please note that if you do not complete the General Complaint form and instead you mail in a general statement or letter, you may be asked to fill out a complaint form in order to provide all the information necessary to create a complaint record.
