What the pseudonym form does
A pseudonym is a different name that will be used instead of your real name in public files about your case. This includes:
- Police reports
- Press releases
- Court records
- Other public documents about the offense
Note: This protection applies only to records about this specific case. Your real name will stay private, while the pseudonym appears in public records.
Who can use a pseudonym
You can use a pseudonym if you're a victim of:
- Sexual assault or other sex offenses
- Stalking
- Family violence
- Human trafficking
How to apply for a pseudonym
Step 1: Download the forms
Step 2: Fill out your part
- Fill in the information requested on the pseudonym form
- Choose which organizations can know your real name and information
- Leave the "To Be Completed by Law Enforcement Agency" section blank
- Sign and date the form
Step 3: Submit the form
Take or send the completed form to the law enforcement agency investigating your case.
They will:
- Replace your real name with your chosen pseudonym in their records
- Tell the prosecutor's office about your choice to use a pseudonym
- Keep your form and real name confidential
- Help protect your privacy throughout the process
What law enforcement will do
Law enforcement has a responsibility to help protect your privacy.
They must:
- Tell you about your right to use a pseudonym
- Explain how the form works and what it does
- Help you fill out the form if you want assistance
- Change all their records to use your pseudonym once you submit the form
- Keep your real name private
- Tell the prosecutor's office about your pseudonym
- Store your information securely to protect your privacy
