The Contracts and Asset Managament Division provides required forms for organizations awarded grant contracts. If your organization has a grant contract with the OAG, these forms were personalized and emailed to your organization. Below are PDF sample documents, instructions, additional forms, and contact information if you have questions or need assistance.
If your organization has a grant contract with the OAG, your personalized Combined Reimbursement forms were emailed to your organization.

If your organization has a OVAG, VCLG, or SAPCS-Federal grant contract with the OAG, your personalized Performance Report forms were emailed to your organization.
Performance Report forms must be completed and emailed to: OAG-Grants@texasattorneygeneral.gov.
If you have not received the personalized financial and programmatic forms or have questions about the forms or instructions, please contact your OAG grant manager.
If you are new to your organization and do not know the name of your grant manager, please email us at OAG-Grants@texasattorneygeneral.gov with your name, title, organization name, phone number and OAG grant contract number, if known.
